UTD Certification Class Student Registration - $25.00

If you are taking a UTD Certification Class you will need to complete the Certification Class Student Registration Process. There is a $25 fee for this. If you bought one of the classroom packages or are a UTD Member it is part of your benefits, if not you will need to pay $25. To begin the process you will first need to Pay your registration Fee and then Complete, Print and Submit a variety of very important forms. These forms are reviewed by Headquarters and your UTD Instructor to ensure that you have the appropriate diving history and medical background to safely take the class. Keep in mind you must bring an unsigned hard copy to your certification class (do not sign them, you will sign them during the first day of class in front of your UTD instructor.)

Note: Only needed if you are a student intending to take a complete UTD Certification Class! This process is NOT needed if you are enrolling in just an Online Classroom!

IN-PERSON CLASS REGISTRATION FORMS - Please complete each document. Remember to print the document before submitting and make sure you bring the unsigned printed copied to the first day of class, to sign in front of your UTD Instructor.
