New UTD Registration Forms and Database

We have just changed all of the UTD registration forms, simplifying and streamlining the UTD class registration process.

Following the links below will take you to the Student Registration Form, Medical History, and Waiver/Release. There is also a new Statement of Completion and Student Feedback form.

The new system will email you a copy of your forms - you will still have to print and sign them in front of your instructor, but it's much cleaner than the old system.

If you have already taken a UTD class, you are welcome to go through the new registration process at any time - it will get you in the system for future classes.

This is a brand new registration database. There may be one or two small bugs still to find, so be patient with us. We have tested it extensively and think everything works.

Please complete each form - registration, medical history, and waiver. Submit and print the documents and bring the unsigned, printed copies to the first day of your class, where you'll sign them in front of your UTD Instructor.
